Dr. Kenneth Cooper developed the 12-minute run test in 1968 for the US Air Force as a fast field test for aerobic capacity. Cover the maximum distance you can in 12 minutes at a sustained hard pace, and the formula converts that distance directly into an estimated VO2 max.

This is a maximal-effort test — pace yourself to finish strong rather than starting too fast and fading, which will underestimate your true aerobic capacity.
